1Before surgery
The 2 to 4 weeks before your operation
Why it helps
Pre-hab exercises build the thigh muscles that support your knee after surgery. A sore knee can make them hard to do, and Pain Care Labs suggests VibraCool for pre-hab pain. Learning to fit it now also means you are not working out straps on day one. In the Columbia University trial, patients were shown how to use it before surgery.
What to do
- Use it for 20 minutes before or after your pre-hab exercises
- Practise strapping it on so you know the fit
- Tell your surgeon and physio you plan to use it
- Freeze both ice packs the night before surgery
2The first days home
From when your nerve block wears off
Why it helps
Pain is usually strongest in the first days. Cold eases pain and swelling, and vibration adds a second signal that competes with pain. In a small, unpublished pilot, ACL patients who used VibraCool from the day of surgery took 35% fewer opioid tablets in the first week than a comparison group.
What to do
- Start once your surgeon agrees and feeling has come back after the nerve block
- Place it on the skin just above where your dressing ends
- Use it with the ice pack for 20 minutes, as often as every 2 hours
- Keep your leg raised while you use it, if your surgeon has advised this
3The rehab weeks
Once your physio exercises start
Why it helps
Sore, stiff muscles are a common reason people fall behind on their exercises. VibraCool is FDA cleared for use before or during physio, to ease muscle pain and tightness. Less pain makes it easier to keep up with your physio plan.
What to do
- Before exercises, use vibration alone or with a hot pack for 5 to 10 minutes to warm stiff muscles
- After exercises, use vibration with the ice pack for 20 minutes
- Use heat with vibration for sore thigh muscles or a tight IT band
- Keep going 2 to 3 times a day until the pain settles
Placement
Where do you put VibraCool after knee surgery?
Put VibraCool on the skin just above where your dressing ends, on the thigh side of your knee. Never put it on the wound or the dressing.
It does not need to touch the wound to help. Vibration and cold travel along the same nerves that carry pain from your knee, and studies show vibration eases pain in nearby areas, not only the spot it touches.
- Slide it a little up or down until you feel the most relief
- Once your wound has closed and your surgeon agrees, place it directly over the sorest spot
- Face the smooth blue side of the ice pack to your skin for stronger cold. Turn it to the soft white side if it feels too cold
- Ask your physio to check the spot at your first post-op visit

How long should you use VibraCool after knee surgery?
Use it for 20 minutes at a time with the ice pack. Pain Care Labs suggests up to every 2 hours in the first days, then 2 to 3 times a day during rehab. Vibration on its own can be used for as long as you like.
Step by step
- Freeze the ice packs solid. Soft, squishy packs soak up the vibration
- Fit the ice pack under the unit and strap it in place
- Press the button. It runs for 10 minutes, then switches off. Press it again for a full 20 minutes
- Take it off after 20 minutes, which is about how long the ice pack stays frozen
- Put the pack back in the freezer for your next session
When not to use it
- On the wound, broken skin or a rash
- If you have calf pain you cannot explain. After surgery this can be a sign of a blood clot, so call your surgeon or GP straight away
- With the ice pack while your leg is still numb from the nerve block, because you cannot feel if it is too cold
- With the ice pack if you are sensitive to cold, for example with Raynaud's or CRPS. Use vibration on its own instead

Knee replacement and ACL recovery: your questions answered
Knee replacement surgery works through bone, muscle and soft tissue, so strong pain in the first weeks is expected. The American Association of Hip and Knee Surgeons recommends combining pain medicines with ice and elevation. Your surgeon sets your medicine plan. At home, ice for 20 to 30 minutes at a time, raising your leg above heart level and the exercises your physio gives you all help. VibraCool adds vibration to cold in one wearable strap.
Pain is strongest in the first weeks and eases as your knee recovers. The NHS says full recovery can take several months or longer, and most people return to work after about 6 to 12 weeks. If your pain gets worse instead of better, or comes with redness, heat or fever, call your surgeon.
Ice and elevation are the standard home measures for swelling after knee replacement. The American Association of Hip and Knee Surgeons suggests ice for 20 to 30 minutes, then off for 30 minutes before reapplying, with a towel between the ice and your skin. Your surgeon will tell you how many weeks to keep going. A VibraCool session is 20 minutes with its own ice pack, so it fits the same routine.
Most people use a mix. Your surgeon manages your medicines. Alongside them, ice, compression, elevation, gentle movement and physio all help. VibraCool is a drug-free option you can add at home. Ice machines and cryo cuffs pump cold water around the knee and are often hired for the first weeks after surgery. They are one option, not the only one. Use whichever your surgeon allows.
